If you using the online form,you will need to submit the form for each item. There is an option to return to form after submitting it to enter the additional items being dropped off.
Notes:
Please DOUBLE check all spelling and capitalization. I will embroider items exactly as shown on the forms.
Shirt Placement –I strongly urge you to place a safety pin (less likely to fall off) or a piece of tape/sticker where you want the embroidery so that it is not too high or too low.
